Therapeutic ultrasound was a physiotherapy mainstay from the 1980s onwards but has fallen out of favour as systematic reviews have struggled to demonstrate consistent benefit over placebo. PEMF therapy has stronger and more consistent evidence for the same conditions — non-union fractures, soft-tissue injury, joint pain, post-surgical recovery. PEMF works at the cell membrane via induced micro-currents; ultrasound works via mechanical sound-wave vibration and tissue heating. Different mechanisms, different evidence trajectories.

Is therapeutic ultrasound the same as the ultrasound used for imaging?

Same physics, different application. Imaging ultrasound is low-intensity to make a picture; therapeutic ultrasound is higher-intensity to heat and stimulate tissue. The therapy variant is what physiotherapists used widely from the 1980s onwards.

Why is therapeutic ultrasound used less today?

Modern systematic reviews have failed to show consistent benefit over placebo for many of the conditions ultrasound was traditionally used for. Many physiotherapists have moved away from it in favour of better-evidenced interventions like PEMF, shockwave, and progressive loading.

Does PEMF have better evidence?

For most conditions where they overlap, yes. PEMF has stronger evidence for non-union fractures, knee osteoarthritis, post-surgical recovery, and a growing range of musculoskeletal indications.

Could I use both?

You could, but most modern protocols don't bother. PEMF, shockwave, or progressive loading exercise is usually preferred.
